2. Read the Agreement for Consulting Services and indicate your agreement by either affixing your electronic signature below or by printing the document, signing it, and mailing it in.

By the electronic signatures below, each applicant agrees that he/she has read and understood the terms and conditions stated in the Agreement for Consulting Services and he/she agrees to be bound by these terms.
3. Submit payment. We accept MasterCard, Visa, and American Express online through our secured order form. You may also pay by check via regular mail.
